Community Aged Care Packages (CACPs) Program
A Community Aged Care Package is a care option that provides an alternative to low level residential care for a frail aged or older person with complex needs. The aim is to enable the person to remain in their own home for as long as possible.
Which area we serve?
We serve the Chinese elderly in the
Eastern Metropolitan Region: Cities of Boroondara, Manningham, Moroondah, Monash, Whitehorse, Yarra Ranges and Knox.
Northern Metropolitan Region: Cities of Banyule, Darebin, Hume, Moreland, Nillumbik, Whittlesea and Yarra.
How do I access a Community Aged Care Package?
Access to the Community Aged Care Package is through the Aged Care Assessment Services (ACAS). Anyone can make referral to ACAS.
Following referral to the ACAS, an Assessment Team member will do an assessment to determine whether the person is eligible for a package of care.
If the person is eligible, the ACAS will approve and send the assessment information to the CACPs Case Manager who will advise if the program currently has the capacity to provide the required care services.
If there are no vacancy at the time of referral, the applicant will be placed on a waiting list and will be advised when a vacancy is available.
If the person is not eligible for a package, the ACAS will advise the person and or the family of other support options that are appropriate for them.
What is the cost?
The cost to client may vary according to the range and frequency of services and their income. The program is funded by the Department of Health and Ageing, together with a weekly fee from the client.
